Saving Has Never Been Easier

Through the Live Better Louisiana program, state employees on qualified health plans can save $120 on health coverage premiums. Live Better Louisiana is an easy-to-use wellness incentive program offered by Catapult Health and the Office of Group Benefits.

It’s as easy as 1-2-3!

1. Beginning in January 2024, visit your local Access2day clinic for an OGB wellness check-up and bring the Catapult Health Primary Care Provider (PCP) form.

2. After your check-up, send the completed PCP form to Catapult Health by deadline of the form. It is important to follow the directions on the form.

3. You’ll save $10 a month on your 2025 health insurance premiums!

It is the member’s responsibility to send the completed PCP form to Catapult Health by the deadline, not the medical provider’s. Only the primary insured is eligible for OGB’s wellness discount.
